Complete annual renewal form and submit with appropriate annual renewal fee plus reinstatement fee of $200 |

Complete Reinstatement of Lapsed Registration form and submit appropriate annual renewal fee plus reinstatement fee of $500 for practising status or $200 for non-practising status |
