Please see below for the reinstatement process in the event your registration (dentist) or certification (CDA) has lapsed effective March 02, 2010.

Complete annual renewal form and submit form with appropriate annual renewal fee plus reinstatement fee of $200 |

Complete Reinstatement of Lapsed Registration form and submit appropriate annual renewal fee plus reinstatement fee of $500 for practising status or $200 for non-practising status |
