Certificate of Standing

Dentists Coming into BC from Out of Province

Dentists planning to move to B.C. from another province require a CDSBC Certificate of Standing Form completed by the regulatory body in their home province. CDSBC must receive the Certificate of Standing as part of the application process.

Please note: You will need Adobe Reader 8 or higher to fill out the form, save it and send it back to us.

You can fill in the form electronically and email it to registration@cdsbc.org, or you can print and submit it via mail to CDSBC.

Here is the process:
  • The dentist who is requesting the CDSBC Certificate of Standing completes the section “Dentist’s Consent for Release of Information,” including signature, witness signature, and date.
  • The dentist submits the form in its entirety (including pages 1-9) and payment to the regulating authority in their home province. Please contact that regulatory authority to find out their fee to complete this form.
  • The regulatory authority in the dentist’s home province completes the remaining sections of the CDSBC Certificate of Standing Form, including signature and seal, and submits the form to the CDSBC Registrar.
